Job Title:   GERIATRIC CARE MANAGER

JOB DESCRIPTION:

LifeStages, a division of VNA Home Health Hospice, has a care management position available for a health professional with experience working in the eldercare environment. You would be responsible for providing services that assist older adults and persons with disabilities in maintaining their independence and attaining their maximum functional potential. Services may include but are not limited to housing transitions, medical management, family communication, home care services and socialization. In addition, this role is an experienced guide, advocate and resource for families of older adults and others with chronic needs.   Utilizing the latest information technologies and telecommunication systems, VNA provides you with a variety of care management experiences and the ability to play an integral role in collaborating with other members of the client care team. 

 


JOB REQUIREMENTS:
Bachelor's degree in counseling, nursing, mental health, social work, psychology or gerontology from an accredited school
Two years supervised experience in gerontology; or certified members (CMC, CCM, C-ASWCM or C-SWCM) may be non-degreed RN (or other degreed professional) with three years of supervised experience in gerontology
Current licensure in designated professional discipline in the state of ME
Current BLS for Healthcare Providers
Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions
Current ME Drivers license
Reliable transportation and Agency required auto liability insurance.
